Please note, you need to be working with an Able Freight Services, LLC account holder (an Able sales or operations employee) to initiate this request.

BY SUBMITTING THIS FORM, YOU, “CUSTOMER”,:
(i) WARRANT THAT YOU HAVE THE RIGHT, AUTHORITY, AND CAPACITY TO ENTER INTO THIS AGREEMENT, ON BEHALF OF YOURSELF AND THE COMPANY, ENTITY OR ORGANIZATION YOU PURPORT TO REPRESENT; AND
(ii) AGREE TO BE BOUND BY THE RELEVANT TERMS AND CONDITIONS OF SERVICE AS MAINTAINED BY ABLE FREIGHT SERVICES, LLC, AS AMENDED AND UPDATED FROM TIME TO TIME AND AVAILABLE OR REFERENCED AT: www.ablefreight.com/customer-resources/general-terms-and-conditions

Customer (You) and Able Freight Services, LLC (the Company), communicate with each other electronically and acknowledge and agree that electronic communication is a proper means for concluding contracts and agree not to contest the validity of contracts on the basis
1) that they were concluded by electronic communication,
2) that the original records are in electronic form, or
3) that no physical signature(s) evidence such contracts.

I/We certify the above information provided in this form is true and correct.

I/We certify that I/we will not tender for shipment any items or commodities that are considered prohibited and/or dangerous by the U.S. Customs and Border Protection, the Transportation and Security Administration (TSA) or any other U.S. government agencies, such as the U.S. Fish and Wildlife Service, the U.S. Department of Agriculture and the Centers for Disease Control and Prevention, or by any other equivalent government agencies in the departure, destination and/or connecting jurisdictions.

I/We have read and I agree to the Terms and Conditions, which can be found at here. In tendering Shipment, the Customer agrees to these Terms and Conditions of Contract.

I/We have read and understood that “49 USC 114" authorizes the collection of this information. The information you provide will be used to qualify you or verify your status as a possible “known shipper.” Providing this information is voluntary, however, failure to provide the information will prevent you from qualifying as a “known shipper.” This information will be disclosed to TSA personnel and contractors or other agents including Indirect Air Carriers (IAC) in the maintenance and operation of the known shipper program.

TSA may share the information with airport operators, foreign air carriers, IACs, law enforcement agencies, and others in accordance with the Privacy Act, 5 USC Section 552a. For additional details, see the system of records notice for Transportation Security Threat Assessment System (DHS/TSA 002) published in the Federal Register.
